Copy of the deceased employee's service book.
Succession Certificate.
Leave Encashment Order.
Obituary/Condolence Order.
First Appointment Order of the deceased employee.
Pension Data Roll Sheet.
Family Registration Certificate (FRC).
Non-Marriage Certificate (where applicable).
Attested copies of CNIC, educational certificates, domicile and recent passport-size photographs.

Important Instructions

Only the legal heirs of deceased employees who died during service are eligible.
Applications must be complete in all respects.
Incomplete applications may be rejected.
False information or forged documents will lead to cancellation of the application.
Applications should be submitted to the concerned DDO Office of the Health Department.
Applications received after the closing date will not be entertained.
